Pre-existing Conditions: Refers to conditions for which you received medical treatment, diagnosis, care, or advice within a specific number of months immediately preceding your effective date. The "pre-ex" period will vary depending on the state in which HCC Life STM is purchased. This exclusion does not apply to a newborn or newly adopted child who is added to coverage.

Consumer Benefits of America: In most states, HCC Life STM is available only to members of the Consumer Benefits of America Association (CBA). Membership in the association will entitle you to discounts of up to 40% off regular retail prices on most short-term and long-term prescription drugs. Discounts are available from over 59,000 participating pharmacy providers nationwide or by mail service. If you are not already a member, enrollment in the association is easy. Details about this membership benefit accompany your HCC Life STM insurance documents. CBA membership and the prescription drug discount card may not apply in all states.

HCC Life STM Plan Exclusions²:

HCC Life STM does not cover: pre-existing conditions including those not inquired about on the enrollment form); preventive or wellness doctor visits; dental or optical treatments; routine physical exams; normal pregnancy or childbirth; well child care; interscholastic and intercollegiate sports injuries; over-the-counter medications and outpatient prescription drugs; expenses incurred outside the United States, its possessions or territories, or Canada.

The following conditions are also excluded: varicose veins, substance abuse and alcoholism, chronic fatigue or pain disorders, and allergies, except for emergency treatment of allergic reactions. Expenses during the first 6 months after the effective date are excluded for (subject to all other coverage provisions, including the pre-existing condition limitation): a. Total or partial hysterectomy, unless it is medically necessary due to a diagnosis of carcinoma; b. Tonsillectomy; c. Adenoidectomy; d. Repair of deviated nasal septum or any type of surgery involving the sinus; e. Myringotomy; f. Tympanotomy; g. Herniorraphy; or h. Cholecystectomy.

RxSavingsPlus Prescription Discount Program:
Membership in the CBA means you will receive additional discounts for new or refill prescription medications at no cost. The average savings is 20%! To save up to 50%, use their mail order program.

The RxSavingsPlusSM prescription discount card is accepted in over 59,000 pharmacies, like CVS, Walgreens, and Rite Aid, across the country. All you have to do is take your prescription to one of the participating pharmacies, show them your prescription discount card and pay the discounted amount.

LEGALCARE America Legal Assistance Program:
The LEGALCARE America Legal Assistance Program is a program offered by the CBA to provide members with access to legal assistance, whether it is for personal or professional reasons. You can get assistance over the phone for things like writing a will, purchasing a large item like a home, having problems with getting a refund or solution from a company, and child support and divorce. The LEGALCARE helpline is free to members.

When you call, you will get to speak to an attorney to get legal advice and information. You may call as often as you need, there is no limit on the number of times you call or the length of your conversation.

If representation is needed, LEGALCARE will also provide you referrals to law firms in your area that specialize in your circumstance. If you choose to use one of the referring law firms, you will receive a complimentary 30 minute consultation and a 25% discount on their standard billing rates.

FTD.com Discount Flowers and Gifts:
FTD.com has teamed up with the CBA to offer its members a 15% discount on their orders for flower arrangements and gifts. You can choose specialty gifts from brands like Build-A-Bear, Mrs. Field's Cookies, Ghirardelli Chocolates, and more.

Budget Truck Rental Discounts:
As part of your membership with the Consumer Benefits of America Association, you will be eligible to receive a 15% discount off of your time and mileage for local and one-way rentals.

North American Van Lines Discounts:
While a member of the CBA, you will receive at least a 50% discount on your rate for interstate moves. You'll also receive a 45% discount on storage. You will be excluded from paying the raised "moving peak season" rates from June to August. Plus, you will receive up to $50,000 in insurance coverage for your personal belongings, free of charge.

When you enroll online, your initial payment must be made by credit card (Visa, American Express, Discover or MasterCard).

If your enrollment is by mail, you may use a personal check or credit card. Payment options include Single Up-front or Monthly payments. There is a small fee when choosing to pay monthly, and monthly premium payments are payable by credit card only.
